The spleen is a fist-sized organ on the upper left side of the abdomen, tucked under the lower ribs behind the stomach. It filters the blood, removes old red blood cells and helps the body fight infection.
Where is spleen pain felt?
Spleen pain is usually felt in the upper left part of the abdomen, under the left ribs. It may feel like an ache, fullness or tenderness. Sometimes the pain spreads to the left shoulder tip, because irritation under the diaphragm is "referred" there. Some people with an enlarged spleen feel full quickly after small meals because the spleen presses on the stomach.
Pain in this area can also come from the stomach, left kidney, pancreas, bowel, ribs or lungs, so the location alone does not confirm a spleen problem.
What is an enlarged spleen?
An enlarged spleen (splenomegaly) is a spleen bigger than normal. It may cause no symptoms and be found during an examination or ultrasound, or it may cause left upper pain, early fullness, tiredness or frequent infections.
Common causes of spleen problems
- Infections such as malaria, typhoid, dengue or viral infections
- Liver conditions that raise pressure in the veins (portal hypertension)
- Blood disorders, including some anaemias
- Injury to the left side of the chest or abdomen
- Cysts, abscess or, rarely, tumours
How is it diagnosed?
A doctor examines the abdomen and may advise blood tests and imaging such as an ultrasound or CT scan. Treatment depends on the underlying cause and is decided after examination. More on our spleen conditions treatment in Hyderabad page.
When to go to the emergency department
Severe left upper abdominal pain after a fall, road accident or sports injury, especially with dizziness, fainting, a fast heartbeat or pain in the left shoulder, can be a sign of internal bleeding. Go to the nearest emergency department immediately.
Protecting an enlarged spleen
If you have been told your spleen is enlarged, ask your doctor before contact sports or heavy lifting, since an enlarged spleen is more easily injured.
